Personal/Professional Information

We have worked hard to streamline this onboarding process to simplify it for you! The link below covers several important items for you to review and documents for us to collect from you. It takes 5-10 minutes if you have the forms handy below.

Note: We will need this updated annually – you will be prompted via email every year to do so.

To complete the form, you’ll need to have or provide information on the items below.

  • Your curriculum vitae (CV), which must include:
      • ✓ Current personal contact information (name, address, cell phone #, email)
      • ✓ Education, experience and/or training that supports the topic you are presenting
      • ✓Any continuing education taken and given in the past five (5) years
      • ✓ Your professional license number(s) and state(s) of licensure
  • Photocopies of all current, active professional license(s): If you have been licensed in multiple states or territories,please provide that information as well.
  • Potential Conflicts of Interest (COI): PESI will use information provided in the Conflict of Interest section of the form to create a speaker disclosure statement that identifies any potential professional, personal, or financial conflicts of interest to participants. This statement will be added to the end of your biography online and in the written brochure. For example:
    • Financial: [Speaker Name] maintains a private practice. She receives a speaking honorarium from PESI, Inc.
    • Non-financial: [Speaker Name] has no relevant non-financial relationship to disclose.

Financial Information/Compensation and Promotion

PESI is committed to do everything we can to promote your work and your content. Along with promoting this program on PESI’s domestic (US) and international websites, we may, at times, also work with other affiliate partners to promote and sell your content.

In promoting your work, PESI maintains the right to use your name, trademarks, image, voice, likeness, and biographical material in connection with promoting your presentation. PESI also maintains the rights to any outlines, marketing and promotional materials related to your presentation, event, or recorded content.

PESI will pay you the agreed upon honorarium (ex gratia payment) for your standard full day speaking
events. Short/other events will be set up event by event. Payments are made via direct deposit the week following the Webcast only 5/2025 live (or recorded) presentation. Your presentation may be broadcast, recorded, re-sold, re-purposed and/or sold on its own or combined with other presentations to create additional trainings, including but not limited to the formats of DVDs, digital seminars, bundles, online courses, certification courses, and summits. PESI shall have the sole right, to determine the price for any presentation, whether in-person, live broadcast, or recorded.

For aftermarket recordings, PESI will calculate your royalty based on net revenue of your recorded presentation(s) and your agreed upon royalty percentage from SPEAKER AGREEMENT. For the purpose of this Agreement, net revenue is defined as all revenues received by or on behalf of PESI less customer returns; and excluding any taxes and sums charged separately to the customer for PESI’s actual out-of-pocket cost of shipping. Royalties are paid once a product is delivered to customers, not on pre-orders. Royalty payments are paid monthly via direct deposit. No royalty will be paid for copies of the recorded product sold or furnished gratis to speaker, distributed gratis for review, distributed gratis to our military customers per PESI’s mission, advertising, promotion, sample, or similar purpose.

If the recorded presentation is sold in combination with other presentations, such as an online course, you will be compensated royalty on your recorded presentation’s percentage of the overall course’s value. For example, if your content makes up 1/3 of the course, you are paid your royalty percentage on 1/3 of whatever the customer paid for the course. If the recorded presentation or online course is sold through an affiliate partner, your royalty will be paid on the amount after the affiliate is paid. For example, if the affiliate partner is being paid 10%, your royalty will be calculated based on the remaining 90% of the sale.

Program Development

At this point, you’ve likely already started a program/topic proposal.

Just a reminder that we’ll need (you can find resources for each of these on www.pesi.com/customer-care/speaker-resources:

    • ✓ Bibliography – due at the time you turn in your final brochure content.
    • ✓ PPT presentation and any handouts you want to provide – draft due 10 weeks prior to your event; final version
      due 4 weeks before your event.
        • ✓ Take time to review the Copyrighted Material and Commercial Bias information in this document.
    • ✓ Post-test questions (attendees complete these after your event to earn CE credit) – due 10 weeks prior to your event.
