Carter Comrie, PhD, LP, is a licensed clinical psychologist based in Southeast Michigan and is the mental health lead for Crossover Health, a tech-enabled, direct-to-employer interdisciplinary clinic system located in the Detroit, Michigan area. In addition to his role within the Crossover, he is the current chairperson for the Michigan Psychological Association (MPA) Pain Psychology Task Force and serves as the vice-chairperson for the Michigan Psychological Association (MPA) Pain Psychology Task Force and serves as the vice-chairperson for the Michigan Health Professional Recovery Committee, being the representative from the board of psychology. He completed his PhD at the University of Detroit Mercy and his post-doctoral fellowship at Ascension Genesys Hospital in the Flint, MI area. In his free time, he enjoys exploring new restaurants, running, and travelling with his family.

James Leonard, DC is a licensed Chiropractor and the market provider manager for Crossover Health, a tech-enabled, direct-to-employer, interdisciplinary clinic system located in Detroit, Michigan. He oversees the implementation and standardization of population-health goals, and actualization of the quadruple aim. He has served on multiple multidisciplinary task forces, including the Utah Coalition for Opioid Overdose Prevention, MIBAC Michigan Back Collective, and the ACA’s Opioid Legislative Action Committee. James is passionate about systems design, intentional care coordination, and making it financially viable to do the right thing.

  1. Investigate the Medical, Physical Health, and Mental Health Disparities that exist within the Black Community.
  2. Review of the extended Biopsychosocial Model.
  3. Describe the roles within an Integrated Health Care System – Highlighting importance of multiple access points and structured internal referral partnerships.
  4. Describe Internal Review of the Impact of Social Detriments of Health and Mental Health Outcomes.
  5. Discuss how healthcare systems can strive to make data actionable through intentional systems design.

Outline

Health Disparities in the Black Community
  • This portion of the presentation will focus on discussing some of the historic health disparities in the Black Community with a focus placed on mental health concerns
    • A patient’s story will be used to highlight the lived experience of the health disparities experienced by the Black Community
      • The goal of this section is to assist the audience to understand the lived experience of these health disparities
Social Determinants of Health
  • This section of the presentation will focus on reviewing the domain of the social determinants of health that have been outlined in the Healthy people 2020 study, discuss how we have implemented a SDOH screening process, and discuss the prevalence of SDOH concerns within the patients treated by the Detroit Mental Health Team
    • The intent of this session will be to highlight the connection between the SDOH and the health disparities and introduce internal SDOH research project
Integrative Care Systems and the Intentional Delivery of Healthcare Interventions
  • In the final section, the presentation will focus on discussing the intentional interventions to address the health disparities experienced.
    • Additionally, there will be a discussion of the impact of the interventions on treatment outcomes
      • The intent of this session is to emphasize the impact of SDOH on clinical outcomes and data informed decision making
        • Through this process, there will the opportunity to discuss how these interventions can be replicated and improved
